When can I look up or use a Plan?
The guiding principal of using CA:tCH Plans is: "When you need to know for the purposes of caring for the participant based on your professional or clinical judgement."
You can use a CA:tCH Plan when:
A Plan is considered "used" anytime it is viewed in the WISHIN Portal or WISHIN Pulse.
You can use a CA:tCH Plan when:
- After immediate safety has been established.
- Responding to a call that may involve mental health.
- A participant or support person requests it.
- The participant is making statements that point to an elevated risk of safety to themselves or others. If the participant has made a statement of intent to self harm.
A Plan is considered "used" anytime it is viewed in the WISHIN Portal or WISHIN Pulse.
What are the steps to remember when using a CA:tCH Plan?
- Check in with are you feeling approaching the situation.
- Communicate with the participant, let them know you have a copy of their plan and you are going to use it to help.
- Be present with the participant and let them know they are safe.
- Validate the experience they are having and refocus to the moment.
- Call a support person listed on the plan, ask if this is still a good person to call.
- Help them to do an activity in their plan.

Support Contacts: what are some tips for contacting them?
- Ask participant if this is still a good person to call and how they would like help from the support person.
- When calling support people let them know that they are listed on the participant’s CA:tCH Plan. Ask if they have time to be a support at this time, if not can they assist later.
- If calling a therapist or doctor, leave a way for them to contact you for more information about the crisis if needed.

Anytime a Plan is viewed it is considered "used" and the reasons are documented as actions in the CA:tCH/WISHIN Portal.

Actions that need to be are documented:
- Plan is Engaged - Plan was used at a time of crisis, to de-escalate a potential crisis, or at participant request.
- Diversion- Participant was referred for diversion support or program.
- Reviewed Plan w/Participant - Plan was reviewed by a facilitator with the participant, no changes were made.
- Reviewed Plan w/Care Team - Plan was reviewed by a participant care team as part of plan engagement/use follow-up
- Reviewed and Updated Plan w/Participant -Facilitator reviewed and updated plan with the participant.
- Contacted Personal Support/s in Plan - During or following use/engagement of plan, personal supports listed in plan were contacted.
- Contacted Agency Support/s in Plan - During or following use/engagement of plan, agency supports listed in plan were contacted.
- Updated Plan at Participant Request - Plan details were updated at participant request (address changes, support changes, etc.)
- Canceled Plan at Participant Request- If the Participant requests that he/she no longer wants a Safety Plan in place.
- Plan not Engaged- Plan was downloaded but not engaged, updated, or reviewed with participant. (Example, pulling up a plan in error or pulling a plan and not needing to engage the plan with a participant.)
Why do I need to document plan use?
Documenting plan use in the CA:tCH/WISHIN Portal helps us communicate about participant care and connections accross the community. It also helps us document who is using plans and why for evaluation and continuous improvement.

Who can view CA:tCH Plans?
Only people who are part of a CA:tCH Partner agency and have completed training, AND are part of a participant's "care team", and Anyone a partcipant chooses to share thier plan with.
What do I do with paper and digital copies of the plan?
Copies of the plan should only be stored in the CA:tCH WISHIN Portal. Paper copies should be shredded or given to the participant. Digital copies should not be retained outside the WISHIN system.

User Permission/Changes
How do I add or remove Users/Facilitators in WISHIN?
Only project leads can authorize new users and facilitators in their organization.
All new users and facilitators must complete CA:tCH Plan Training as outlined in the partner MOU, before receiving a log in for the WISHIN / CA:tCH Portal. If a user or faciltator has left your organization please contact WISHIN to remove them from the system.
